Open the template, add a new layer as close to the bottom as possible, flood-fill white and move down. While still on this layer, go to Image > Canvas size > 

Move the word at to the bottom of the template and then above all layers. Move the rectangle below both star layers.

Open Beachy Stars and Stripes papers 36 and 40. Use paper 40, after a 75% resize, to replace the red oval and paper 36, after a 50% resize, to replace the dark oval.

Use paper 36 to replace both frame layers. Then undo resize, resize to 75% and replace the dark star layer.

Using colour DB0908 to replace the the word art. Add eye candy glass to the word art. 

Open paper 22, resize by 65% and replace both back frames for both frame openings.

Use paper 43, after a 70% resize, and replace all stars. Use paper 32, after a 55% resize, to replace the rectangle.

Open element 89, resize by 50% and place above the paper 40 layer. 

Open element 167, resize by 50% and add above the rectangle layer. When  duplicated this layer, I flipped and mirrored and THEN positioned the right hand side.

Add your flower of choice on either side of the design, above the rectangle. Add any other elements you want now as well.
