Open the template and delete the copyright information.

Open paper 1, resize by 20% and replace layer 3, 4 and 6. Open paper 6, resize by 20% and replace layer 1.

Open paper 2, resize by 20% and replace layer 2. Open paper 3, resize by 20% and replace layer 5.

Open the tube and crop out the standing tube, and then resize by 55%. Position as shown in the tutorial, duplicate 2x and then move one above the paper 3 layer (remove excess), above the paper 1 layer (remove excess) and then leave the original above all layers.

Drop shadow the top layer of the tube and then erase the excess.

Open element 18, resize by 50% and add in the lower right hand corner.

Open element 1, resize by 30% and add under the element 18 layer (twice).

Open element 11, resize by 20% and then add below the element 1 layer.

Open element 12, resize by 20% and add above element 1 on either side of element 18.

Add element 14 and add above the layer 7 layer.

Open element7, resize by 18% and add above the element 12 layer (lower).

Open element 4, resize by 10% and add above the element 12 layer (right).

Open element 10, resize by 25% and then add above the element 14 layer; duplicate, move above the tube layer, drop shadow and then remove the excess above the tube's arm.

Open element 16, resize by 70%, mirror and then add below the element 11 layer.

I used paper 5 with my mask but this mask is no longer available. Find a considerable one that's more rounded to get the same look.
